These forms need to be printed, signed, and returned to Campion Academy before Move-In Day to avoid long lines!

New Students

Work Forms

Medical Forms:

Medical Consent Form (requires parent/guardian signature)
Certificate of Immunization (requires doctor's signature)
-or- Certificate of Immunization Exemption
Sports Physical (requires doctor's signature and must be completed after May 25, 2024)
Over the Counter Medication (requires doctor's signature and must be completed after May 25, 2024)

Other documents:

Copy of Social Security Card
Copy of photo ID
Copy of Insurance card

Returning Students

Medical Forms:
Completed annually

Medical Consent Form (requires parent/guardian signature)
Sports Physical (requires doctor's signature and must be completed after May 25, 2024)
Over the Counter Medication (requires doctor's signature and must be completed after May 25, 2024)
